Tom :I will put him on the phone,Just a second.
我会让他接你的电话。“put sb on the phone”意思就是让某人接电话
Jack :Thanks.

如果是你要帮对方转接分机可以这样说：
I am transferring your call 或者说 I'm redirecting your call
